- Full name as on passport
- Date of birth*
- Citizenship
- Passport number
- Date of issue*
- Place of issue*
- Expiry date
- Date of arrival
- Date of departure
- Personal fax number
- Fax number of nearest Italian embassy or consulate in your country
Please specify if the consulate needs also an original invitation. In that
case also send full personal address where to send the invitation to. See
you in Cabella!
